Researchers have known for some time that exercise is good for aging brains. The latest studies, though, are adding some finer points to that message: While exercise appears to work best before memory fades, it also benefits people who've already gotten dementia. And it seems to help not just with Alzheimer's disease, but also with vascular dementia, a kind of memory loss that's caused by "silent" strokes in the brain.
Three studies presented at the recent meeting of the Alzheimer's Association in Washington D.C. detailed the effects. Exercise lowered levels of toxic tau proteins and increased blood flow in the brains of people with early memory changes that put them at risk for dementia. Four months of intense exercise improved symptoms like anxiety, irritability, and depression in people with Alzheimer's, though it didn't help their memories. But 6 months of exercise did improve memory and thinking in people diagnosed with vascular dementia.
Experts say these are important gains for patients who have few effective treatments.
"No currently approved medication can rival these effects," says researcher Laura Baker, PhD, an associate professor of gerontology and geriatric medicine at the Wake Forest School of Medicine.
The Powerful Protection of Exercise
"Exercise is, right now, the most powerful effect in terms of prevention of Alzheimer's disease," says Stephen Rao, PhD, the director of the Schey Center for Cognitive Neuroimaging at the Cleveland Clinic.
"I'm sure a drug is going to come along that will also work in terms of prevention. But right now, the best thing you can do to prevent Alzheimer's is to get people on a treadmill or walking," he says.
Rao has been studying older adults who carry a gene that greatly raises their risk for Alzheimer's disease. About 1 in 5 Americans carry at least one copy of this gene, called APOE4, and it boosts their risk of getting the disease from 3- to 15-fold, depending on whether they have one or two copies of it. When Rao's team started the studies, his volunteers showed no signs of memory or thinking troubles.
New research out in the The BMJ yesterday suggests that the spice of life may be - well, spice. At least when it comes to longevity. The large-scale study that included almost half a million people found that those who had more spicy food - generally in the form of chili peppers - more than once a week had a reduced overall risk of death over the seven-year study period. They also had reduced risk of death from certain diseases like cancer and ischemic heart disease. Though cause-and-effect can't be shown in a study like this one, there's good existing evidence that some of the compounds in chili peppers, namely capsaicin, have some important health benefits, and might be behind the connection.
The researchers at Harvard's T. H. Chan School of Public Health and the Chinese Academy of Medical Sciences looked at data from 485,000 people in China who reported on dietary habits, including how often they ate spicy foods, red meat, alcohol, and vegetables. They excluded anyone who had a history of heart disease, cancer, or diabetes. At the end of the seven-year study period, the researchers looked to see if there were any connections between diet, disease, and mortality risk.
They found that people who ate spicy foods one or two days per week had a 10% reduced risk of overall mortality, compared to those who had a spicy meal less often than once per week. Eating spicy food more than two days per week was only linked to a slight additional increase: Those who ate spicy foods three to five times and six to seven times per week all had a 14% reduced risk of dying. People who didn't drink alcohol seemed to have the most benefit.
When they looked at death from specific causes, they found that spicy foods were linked to reduced risk of death from cancer, ischaemic heart disease, and respiratory system diseases. These benefits were slightly greater for women than men.
Where was the spice coming from? It generally came from some form of chili peppers, and the team also wanted to determine whether eating dried or fresh ones made a difference. They found there was a slight one: Those who consumed fresh chili, as opposed to dried, tended to have a lower risk of death from cancer, ischemic heart disease, and diabetes.
Again, cause and effect can't be "proven" here, it's just a correlation. But if there is a causal link, there are certainly some strong possibilities for how it works. A compound called capsaicin is the active ingredient in chili peppers, and has been linked to an array of biological benefits in earlier studies. Also, fresh chilis are high (higher than dried ones) in vitamins C, A, K, B6, and potassium.
"Spicy food or its active components have been related to improved inflammation, reduced body adiposity, or improved lipid profiles," says study author Lu Qi. "In addition, spicy foods may also affect gut bacteria which has been related to various chronic diseases such as diabetes, cardiovascular disease and obesity. However, we know little about why these may occur. More studies are definitely needed to clarify the mechanisms."
The team tried to control for variables like age, marital status, education, and physical activity, but it's certainly possible that something else might be going on to explain the connection. It could be that spicy food is just a marker for other lifestyle choices that lead to good health.
Still, the connection is feasible, given the existing evidence on capsaicin. And it might explain why, despite how burningly painful spicy food can be, it's pretty ubiquitous in most parts of the world, and some people just can't seem to get enough of it. The Urban Institute, a nonprofit organization closely watching the rollout of health insurance on the health law's exchanges, this week released a new report comparing premiums of the low cost "silver plans" across 30 states in more than 70 "rating regions" for 2014 and this year. Silver plans have been the most popular choice of individuals purchasing coverage on exchanges.
Though some of the nation's largest publicly-trade insurers like UnitedHealth Group (UNH), Cigna (CI) and Aetna (AET) entered more markets in 2015 - the second year subsidized coverage was offered on the exchanges - these large carriers "continue to reflect an aversion to risk by having higher than average premiums," authors of the Urban Institute report said. Meanwhile, many Blue Cross and Blue Shield plans, including those owned by Anthem (ANTM) have kept premiums low with "small increases from year to year."
"They seem to be extremely risk averse," one of the report's authors, Institute fellow John Holahan, said in an interview about the large publicly-traded carriers. "United, for example, came into a lot more markets in 2015 and was rarely one of the low cost plans."
A key reason Blue Cross and Blue Shield plans may have been priced more competitively is their recognizable name and they tend to be the dominant carrier in many markets across the country. "They have the brand name," Holahan said.
The report's authors said Blue Cross plans are "highly competitive in many regions" in part because they have a "near monopoly status with few competitors" and offer "seperate limited network products to allow them to compete in the marketplaces."
But the coming consolidation among the so-called "Big Five" publicly-traded health plans could make them more competitive with regional Blue Cross plans, Urban Institute researchers said.
